The sheathe alerter uses your gear interface to see if you're skilling or not. The game will change your stance when you're skilling to prevent showing a tool twice on your avatar.
You have to use different settings depending on what tool you're using. For some tools you also have to use an off-hand weapon or an override to make the difference visible.
Use the table below to see what settings to use. Set your stance in-game to be the same as the "Idle stance" column, also set this stance in the alerter settings. Now make sure your gear interface is open and turn your avatar to the left and make sure that something sticks out into the marked rectangle whenever you're unsheated.
Note: you need to have your interfaces fully opaque for this to work.
