Alt1 modification and safety

I've read a lot about the safety of Alt1.  There are two aspects that continue to concern me and I'd like to see some sort of evidence that they cant be exploited by malicious operators.  |Alt1 is a screen reader (with restrictions), I do however type in my runscape pin as a visible act, fine if the software cant not be exploited.  Alt1 is open to modification.  Those two features together seem to make this a little risky, not just in terms of theft of runescape accounts but in terms of software modification to act beyond the app. Run as is unaltered its OK...but it can be altered, can safety be assured, the risks are pretty big.

You know you can use both for even higher security? You're just stupid if you don't use a pin. People can still get hacked with 2FA with interception methods.
